Livorno hands you the cruise's biggest fork in the road. Florence is the heavyweight: Renaissance ground zero, 90 minutes each way, magnificent and exhausting, where the honest port-day version is piazzas, the Duomo's exterior and one great museum booked ahead, not all of them. Pisa is the efficient crowd-pleaser: 20 minutes away, tower leaning as advertised, two hours round trip done well.
And Livorno itself is the sleeper: a canal-veined Medici port with Italy's best central market and a fish stew (cacciucco) with five C's and four centuries of attitude. Families with young kids: Pisa plus Livorno beats a Florence forced march every single time.
Livorno is the upstart of Tuscan cities, built almost from scratch by the Medici in the 1590s as their ideal port, complete with canals (the Venezia Nuova quarter still floats on them) and, remarkably for the age, laws inviting merchants of every faith and nation to settle freely. The result was one of the Mediterranean's most cosmopolitan cities: English, Greek, Jewish, Armenian and Dutch quarters trading side by side. Florence, its overlord up the Arno, needs less introduction: the city where banking money met genius and bankrolled the Renaissance, where Brunelleschi domed the impossible cathedral, Michelangelo carved David from a rejected block, and the Medici collected until their last heiress willed the whole lot to the city forever, which is why the Uffizi exists.
Pisa's tower, meanwhile, is a 12th-century bell tower on bad soil that started leaning at storey three and never stopped; engineers stabilised it in the 1990s by removing soil from the high side, settling it at a rakish 4 degrees, good, they say, for another 200 years. Galileo, Pisa's most famous son, allegedly dropped weights from it to test gravity. The photo op was inevitable.

Tuscany runs on regional pride: Livornese cooking is seafood and swagger, Florentine is beef and restraint, and suggesting they're interchangeable is a small war crime. Coffee standing, cappuccino before noon, dinner from 7:30. Validate every train ticket in the green machines or budget for the fine. Florence's Duomo enforces dress codes; its museums enforce timed slots. And Livorno's market etiquette: point, smile, accept whatever extra the stallholder adds, that's affection.

The Medici sea fortress guarding the old harbour, built over a Roman castrum and a medieval tower: ramparts, exhibitions and harbour views, usually free or near it. Your first and last sight from the ship deserves the twenty-minute walk-through it earns.

Livorno's icon: Grand Duke Ferdinando above four extraordinary bronze captives (1626), the city's most argued-about and most photographed artwork. Local legend says find the angle where all four noses align and make a wish. Two minutes, one photo, one wish.

Bus from the centre + historic funicular up; ~40 min each way
Livorno's hilltop sanctuary of the Madonna delle Grazie, patroness of Tuscany, reached by a little funicular: gilded church, a gallery of centuries of sailors' votive paintings (shipwrecks survived, gratitude rendered in oils), and the coast unrolled below. The local pilgrimage cruisers never find.

The lawn of miracles: cathedral, baptistery and that tower, all marble, all tilting photogenically. The square is free and the forced-perspective photo mandatory; the tower climb runs ~€20-27 timed (kids under 8 not permitted up). Two hours door to door done briskly.
