Civitavecchia exists, for cruisers, as Rome's front door, and the good news is the door is cheap: a free port shuttle to the gate, a short walk to the station, and the regional train at a flat €4.60 into the city, no booking needed, 2-3 trains an hour. The great judgement call is scope.
Rome rewards restraint: pick ONE anchor (Vatican or Colosseum), book it timed, ring it with free wonders (Pantheon, Trevi, Navona), eat one unhurried plate of pasta, and leave a fat buffer. The disasters all start with people trying to do both anchors on one port day. And the sleeper option nobody takes: skipping Rome for Civitavecchia's own fort, market and seafront, which is a genuinely pleasant small-city day for the Rome-weary.
Civitavecchia has been Rome's harbour for nineteen centuries: Emperor Trajan built it around 106 AD, and his breakwater still shelters the port your ship ties up in. Michelangelo designed the fortress by the harbour (his last architectural work), which is why it's Forte Michelangelo. Rome itself needs no introduction, but the port-day framing helps: everything you'll see stacks in layers, from the Republic's forum through the emperors' Colosseum (80 AD, capacity 50,000, naval battles staged in the flooded arena during its opening games) to the popes' Vatican, whose basilica stands over what tradition holds is Peter's grave, and whose museums end in Michelangelo's Sistine ceiling, painted mostly lying on his back and complaining about it in verse.
The Trevi Fountain's coin ritual (right hand over left shoulder) funds Roman charities to the tune of over a million euros a year; the Pantheon's concrete dome, two millennia old, remains the world's largest unreinforced one. Rome wasn't built in a day, and the local joke is that it can't be seen in one either. It can, though, be tasted.

Rome is churches and dress codes: shoulders and knees covered for St Peter's and most basilicas, enforced. Coffee is drunk standing (sitting doubles the price), cappuccino is a morning drink, and a €1.20 espresso at the bank of a bar is the city's great equaliser. Water flows free and cold from the nasoni street fountains: bring a bottle. Tipping is rounding up, not percentages. Watch for the standard tourist-zone hustles: 'free' bracelets, gladiator photo demands, and taxi meters that forget to start. Validate train tickets or ride with the €50 fine story instead.
